Fill out the form completely. Please note that by filling out this form you are giving us permission to email you back, if necessary. Press the submit button when you have completed your order and an email confirmation will be sent once your order has been received. You will have to pick up your order at the Student Health and Wellness Center within two weeks of order confirmation.
Condom Prices:
$4 for 12 condoms
You can combine any style condom to total 12 except for the Assortment Sampler
Maximum per order is 48 condoms

We will accept cash and J-cash for orders under $8.00 and MasterCard and Visa for any order over $8.00.
